Step 1. Schedule in-take appointment by calling (818) 999-2800 Ext. 456 or e-mail us.
Step 2. Download following items.
- Intake Form – this form must be completed.
- Appointment Letter – please bring listed documents with you to Intake meeting.
These items must be brought to your intake meeting.
Please allow 2 hours for your In-Take meeting.
